General Office Policies

At this time I am meeting with patients through Telehealth and in person, provided that safety guidelines are met. If you test positive for COVID, the sessions will have to be conducted virtually.

Following CDC guidelines, all safety protocols adhered to by my office are outlined on this Informed Consent Form.

Appointments and Cancellations

Appointments are primarily scheduled via email. Scheduling via phone is also possible. I check messages each business day until 9pm.

Individual sessions are 45 minutes and couples as well as parent guidance sessions are typically 60 minutes. Sessions that require additional time may be prorated based on my hourly rate.

Individuals and families will be charged full session fees if cancellations are not made within two business days – a 48-hour window prior to your appointment time. For example, an appointment scheduled for Monday at 2 pm would have to be cancelled by Thursday at 2 pm in order for a charge to not be incurred.

Medical Insurance

I am an out-of-network provider. Individuals and families are responsible for full payment of all fees for therapeutic services. I will provide an invoice that can be submitted to your medical insurance provider for reimbursement.

Payment

Fees for initial consultations are due at the time of service.

Accepted forms of payment include personal checks, credit cards, Zelle and cash.

Credit card information may be kept on file and billed automatically at the individual or family’s request. Credit card payments will include a 3% service fee.

An invoice will be sent at the end of each month via email. Payments made by Zelle and Credit Cards are due after you receive your invoice on the last day of the month and checks must be received within 5 business days of receiving your invoice.

A complete list of all office policies, including communications and confidentiality will be provided for your review and signing prior to our first session.
